PHTLS promotes excellence in trauma patient management through global education of all providers involved in the delivery of prehospital care. PHTLS is developed by the National Association of Emergency Medical Technicians in cooperation with the American College of Surgeons Committee on Trauma.

PHTLS courses improve the quality of trauma care in your area and decrease mortality. The program is based on a prehospital trauma care philosophy, stressing the treatment of the multi-system trauma patient as a unique entity with specific needs. This may require an approach to the trauma patient that varies from traditional treatment modalities.

PHTLS promotes critical thinking as the foundation for providing quality care. It is based on the belief that, given a good fund of knowledge and key principles, EMS providers are capable of making reasoned decisions regarding patient care. The PHTLS course is continuously updated and revised to keep up with the advances in the field, ATLS Guidelines and feedback from PHTLS participants.

Although PHTLS originated in the United States, it has evolved and now serves as an international education program available to EMS providers in 33 countries including Argentina, Australia, Austria, Bolivia, Brazil Canada, Colombia, Costa Rica, Denmark, France, Germany, Greece, Hong Kong, Ireland, Italy, Luxembourg, Mexico, Netherlands, New Zealand, Norway, Oman, Panama, Peru, Philippines, Poland, Portugal, Saudi Arabia, Spain, Sweden, Switzerland, Trinidad & Tobago, United Arab Emirates (UAE), United Kingdom, Uruguay, and Venezuela.

With temperatures predicted to be 90-100 degrees this coming week, the Rainier A shift crew spent part of the morning refreshing on the operation of our "Type VI" brush rig. This small, agile engine has the ability to travel roads and open areas which are ordinarily not able to be accessed by our larger "Type I" fire engines. The forecasted weather drastically increases the liklihood of us being called to fight a wildland fire.

Our Purple Smoke Tree at the St.Helens Station is in bloom. This tree was donated by the St. Heelns Garden Club a few years ago.

The actual name for this plant is the Cotinus coggygria 'Royal Purple'.

Cotinus gets its common names, smoke bush or smoke tree, from its panicles of spent flowers that appear like puffs of smoke around the plant. It is easy to see why Great Plant Picks chose this shrub for their 2005 list. As summer draws near, the leaves turn purple, almost black, a nice contrast to the red veins and edging of the leaf margins. ‘Royal Purple’ has leaves which change color throughout the seasons, starting out in spring sporting its new fresh attire of maroon-red. In fall, its fashion statement appears in shades of scarlet.

Use fireworks outdoors only.
Obey local laws. If fireworks are not legal where you live, do not use them.
Always have water handy. (A hose or bucket).
Only use fireworks as intended. Don't try to alter them or combine them.
Never relight a "dud" firework. Wait 20 minutes and then soak it in a bucket of water.
Use common sense. Spectators should keep a safe distance from the shooter and the shooter should wear safety glasses.
Alcohol and fireworks do not mix. Have a "designated shooter."
Only persons over the age of 12 should be allowed to handle sparklers of any type.
Do not ever use homemade fireworks of illegal explosives: They can kill you! Report illegal explosives to the fire or police department in your community.

The Rainier station hosted a National Incident Management System ICS-300 class the past two days. Sheriffs Office personnel, CRFR and Oregon National Guard members were in attendance.

The ICS 300 class is designed for responders to work across jurisdictional boundaries, speaking common terminology and developing operational plans when dealing with large-scale disasters.

Today, crews from A shift were given a demonstration of the new Zoll "E" series heart monitor/ defibrillator. Company representative John Phelps gave an excellent presentation on this new lifesaving tool. CRFR hopes to upgrade our outdated models soon, and the E series is a fantastic machine.

On October 8th, the Rainier and Goble crews responded to a MVC. A pick-up truck and ATV collided head-on, critically injuring the ATV rider. E-494 and M-491 crews performed patient care ,while E-491 set up a Landing Zone for Life Flight One at the Goble Fire station.

The patient was packaged and transported to the landing zone, and then flown to a Portland area trauma facility, where he underwent surgery.

The lifesaving efforts and outstanding performance put forth by all involved are commendable.

Rainier Crews Tackle challenging MVC

Today, Rainier Engine 491 responded to a Level-4 MVC on Rainier Hill. A U-Haul truck and trailer lost control heading westbound and jacknifed. The truck ended up on the guardrail. A challenging traffic control situation was easily handled by Capt. Coombs and FF Boles. ODOT and a local wrecker were involved in removing the hazard.

Fire on the top of a Power Pole

This was a fire that started on top of a Power pole. The fire caused a small brush fire around the bottom of the pole.

I am told that these kind of fires are very rare. I was also told that pole top fires can be caused by dust on overhead power line insulators, in combination with light rain and high humidity. This can result in a high level of leakage current across the insulators. The leakage of currents can generate enough heat to ignite the wood at the point of contact between wood and metal surfaces and start a pole top fire.

The Fire was extinguished and the pole was cut just below the fire and the insulators were replaced.

Three "A" shift CRFR firefighters (Coombs, Cole and Brody), in addition to Gorsuch, Wonderly, Barbouletos and Davis completed a 40 hour Confined Space Rescue class last week. We spent 3 days at the training center, and the last day at the Longview sewage treatment plant.

Training was done with Longview Fire, forging a new working relationship with our brothers across the river.

Blowout on Squirt 491

While returning from a commercial fire in St Helens, the A shift crew heard and felt a large bang coming from under Squirt 491. After pulling over, we saw that the right, inside rear tire had blown out. It did a surprising amount of damage to the squirt. The inside fender had buckled and part of the outside body had also buckled.

The apparatus performed safely on one tire and the crew was never in danger.

KGW Channel 8 interviewed Matt Denley today in regards to the childbirth at Meadow Park nursing home. The ever-so-charming Matt did a fantastic and professional job with the interview. Keely Chalmers and her cameraman spent about 30 minutes with Matt. In addition to the interview, he demonstrated some of our pediatric EMS equipment aboard M-471. The timing was perfect, because right at the end of the interview, we popped a call, and the news crew got a shot of Aaron Peterson and Matt heading out to save another life.

The quick extinguishment of a fire by A-Shift saved a garage at the St.Helens Auto Center. Electrical reasons caused a fire in a wall. The fire was contained to a small area in the wall of a Garage used for storage.

The volunteer organization, with the assistance of the paid crews have been busy preparing the station for the arrival and food and toys. Firefighters received a 911 call at about 6 a.m. Tuesday reporting smoke coming from Bing’s restaurant on Columbia River Highway. Flames where comming from the front of the building. The fire took about an hour to contain.

CRFR crews have been very busy the past few days, assisting in recovery efforts in the Vernonia area. The devastation there is incredible. It has hit us close to home, with C shift's Ben Davis home having been severely damaged. A shift personnel (along with the other two shifts and volunteers) have been helping Ben, along with the other citizens of Vernonia. We are doing our job in our usual professional manner.

Our thoughts and prayers go out to those who have lost so much.

I don't think these two guys get enough credit. I mean these two stellar guys have to maintain and repair all the 'stuff' that we attempt to destroy. We're firefighters, after all- we break things. No, we don't try and do it on purpose, but these things just happen. We have to use our tools and equipment in extreme conditions, and even though we have the best of the best, things still break.

That's where Rick and Duane come in. Our district mechanics are reliable, intuitive, creative and charismatic. They are good stewards of the taxpayers money, in that they are not wasteful. If it is possible to be repaired, rather than replaced- they will do it.

These two guys work "behind the scenes", making sure our equipment is up to snuff, and ready to use. When we roll up on an alarm and begin to restore someone's life back to normalcy, it is only because Rick and Duane have made sure our equipment is ready to do it's job.

My hat is off to them.

McCormick Park Apartments Fire on A Shift

St. Helens, OR - - Just before 12:30 A.M. Sunday morning, September 30, 2007, Columbia River Fire and Rescue responded to the report of a fire at the McCormick Park Apartments at 1691 Old Portland Road in St. Helens.

Firefighters arrived to find smoke, steam and water coming from the laundry room of the facility on the second floor. Much of the fire was extinguished prior to CRF&R arrival due to a water hose in the room that burst. The complex was evacuated while firefighters extinguished the fire and checked to make sure flames had not spread to adjoining rooms and apartments. Firefighters allowed residents to return to their apartments a short time later and remained on-scene to check units for working smoke alarms.Fifteen personnel arrived in five engines and two support vehicles. The scene was cleared at 1:48 AM. Damage was limited to the room of origin. No injuries were reported.

Hennigan "smokes" month three test

Tim Hennigan did a great job today on his month three probationary practical exam. Tim's skills and knowledge are obvioulsy increasing every day. We are lucky to have him here with us at CRFR.

After the test, Tim even offered to help out training one of our newer volunteers.. He stood by Ashley Elkins as she took on the ventilation prop for the first time.
